Send your request in writing to the company, not verbally, asking them when they will have this done. Also point out that you have submitted the forms already and still notice that the changes have not been made. Send copies of the forms you filled in with your letter. Then fax (dont drop in person) your letter and copies of the forms to your work so that you have a receipt of your fax transmission.

If they don't respond then write to the president and send him the same material all over again. Then failing that threaten legal action in small claims court where you don't need a lawyer.

The problem may be as simple as some feminist secretary who figures she is going to help your ex shaft you and hope that you die before you manage to get the forms changed at your work.
