Thanks for the information Ed. Guess what! They have absolutely NO RECORDS of me having been there three times over the past 2 years. All the forms I signed, copies of certain pages from my divorce certificate, everything has gone missing! As far as they know if I died while at work, EVERYTHING would have gone to my x as she was still listed as my PRIMARY beneficiary!

This time, thanks to your valuable input, things will be different. It also helps to mention that my lawyer is on top of this and requests signed documents that they are in receipt of all the paperwork. The payroll lady was very nice actually ( not the same person I dealt with prior ) and she spent a good 20 minutes looking for the forms. She came back shaking her head in disbelief, saying "I don't believe it" !!! Perhaps you were on to something Ed! Thanks for responding to my post.

This should serve as a warning to all you single people out there who think "all is in order" after having dropped off proper forms, signed documents at your payroll! Better get a signed receipt that your payroll has in fact accepted your changes and have made the changes according to your desires. Make sure it is dated!
